Medical Fitness Certificates, The One Document With No Workaround at Sign-On

Validity has to cover the full contract period, and the issuing practitioner's recognition status matters as much as the certificate itself.

Current-fact control: This article explains planning and agency practice. Port notices, tariffs, drafts, berth/terminal limits, Customs/Immigration routes, carrier acceptance and technical-provider requirements can change. Confirm the current primary-source instruction for the actual vessel, cargo, equipment and date before operational reliance.
Matarbari scope control: CPA has no standing Matarbari operating desk. Commercial-port plans and design figures are not current open-port availability. Any attendance requires a confirmed facility, project or vessel assignment and current operator/facility acceptance.

A seafarer who arrives at Chittagong for sign-on without a valid, properly recognized medical fitness certificate isn't joining the vessel that day, full stop. This is one of the few documentation requirements where there's genuinely no workaround if it's missing or expired, and manning companies who treat it as just another item on a checklist rather than a hard gate are the ones who end up with a seafarer stuck at the airport. For an actual case, nationality, passport and seaman’s-book status, airline routing, Immigration or Shipping Office requirements, vessel position, weather and safe launch access may change the document route and timing. Immigration, manning and welfare steps are case-specific; confirm current local practice for the crew and ETA.

What applicable seafarer employment and welfare standards actually requires

Every seafarer needs a valid medical certificate confirming fitness for the duties they're going to perform, issued by a duly qualified medical practitioner recognized for this purpose, and the certificate has to be current, these certificates carry validity periods (typically up to two years for most seafarers, shorter for under-18s) and an expired certificate is functionally the same as no certificate at all for sign-on purposes.

Where Bangladeshi seafarers get this done

For Bangladeshi crew, pre-employment medical examinations need to go through medical practitioners and facilities recognized by the Department of Shipping for this specific purpose, not just any general practitioner or hospital. A manning company recruiting Bangladeshi seafarers should confirm the examining facility's recognition status directly rather than accepting a certificate at face value, the same verification discipline that applies to STCW certification we've covered elsewhere on this blog.

International crew joining at a Bangladesh port

For non-Bangladeshi seafarers joining a vessel at Chittagong, Mongla or Matarbari, the medical certificate needs to have been obtained before departure from their home country or wherever their manning company arranges examinations, there's no equivalent on-arrival medical certification process at a Bangladesh port the way there might be for, say, a COVID-era rapid test arrangement. Confirm certificate validity covers the entire upcoming contract period before booking travel, not just that it's valid on the day of departure.

What actually catches manning companies out

A certificate that's valid on departure day but expires partway through the planned contract period, this needs renewal arranged before the gap, not discovered mid-voyage. A certificate issued by a practitioner or facility that isn't actually recognized under the applicable flag-state and employment requirements, which can surface as a problem during a PSC inspection even though the seafarer is physically fit and the paperwork looks fine superficially. And basic mismatches between the medical certificate's stated fitness category and the actual role the seafarer is signing on for.

What we'd recommend checking before every sign-on

Certificate validity covering the full intended contract period, not just the joining date. Issuing practitioner or facility recognition status confirmed, particularly for Bangladeshi crew where Department of Shipping recognition specifically matters. And these checks done with enough lead time that a gap can actually be fixed before travel, not discovered at the airport when there's no time left to do anything about it.

Confirming Medical Certificate Requirements Before Sign-On?

Send vessel name and IMO number, port or anchorage, ETA/ETD, sign-on and sign-off count, nationalities, passport and seaman’s-book validity, flight routing and any medical or visa issue. The duty desk will confirm the applicable Immigration, airline, launch and transfer route before issuing a vessel-specific estimate.
